Boundary spanning for health - a framework and model to improve access to health services in high income countries

This study provides a practical response to the problem faced by health services to engage with hardly reached people to reduce health inequalities and inequities. Evidence from an international two-case study in Australia and Ireland was used to develop the Community Connector Framework. This new conceptual framework includes a Model for Collaboration between health services and community connectors to cross boundaries and engage effectively, resulting in better access to health services, better service provision and ultimately better health.
